Scheduling
When you contact the office of Dr. David Weinstock, a member of our friendly, professional staff will arrange an appointment time that is convenient to your schedule. Our office hours are Tuesday, Wednesday, Friday, from 8:00 am to 5:00 pm, and Saturdays by appointment. Your Initial Consultation
Your initial appointment will consist of a comprehensive oral examination and the collection of diagnostic information. An FMX, full mouth series, of X-rays are usually necessary to make a proper evaluation. During the consultation visit, Dr. Weinstock will explain your diagnosis and develop treatment options. Occasionally, treatment can be performed at the initial appointment. A dental prophylaxis (cleaning) is sometimes possible on the time of the initial consultation visit. However, a complex medical history or treatment plan will require a more in-depth consultation and a second appointment may be necessary to provide treatment options.
Please assist us by providing the following information at the time of your initial appointment:
Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g., diabetes, high blood pressure, mitral valve prolapse, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).
Financial Policies
We are committed to providing you with the best possible care. If you have dental insurance, we are anxious to help you receive your maximum benefits. We will gladly submit your dental claim for you so that you will receive your dental benefits. In order to achieve these goals, we need your assistance and your understanding of our payment policy.
Payments for dental services are due at the time the services are rendered.
As a convenience to you, we offer a number of payment options:
Ask our front office staff about how alternative payment options can allow you to have the healthy smile that you deserve. Returned checks and balances older than 30 days may be subject to additional collection fees and interest charges of 1.5 percent per month.
Insurance Information
We will be happy to help you process your insurance claim form for your reimbursement. We do not participate with any insurance plans, so our financial contract is with you, our patient. Our office will assist in the processing of your insurance claim to the extent that time permits. It is important to understand that dental insurance is designed to reduce cost, not to eliminate it entirely.
Not all services are a covered benefit in all contracts. Some insurance companies arbitrarily select certain services that they will not cover.
When filling out your claim form, please leave the authorization of payment to the dentist signature area blank. The insurance company will then mail the reimbursement check directly to you.
We must emphasize that as dental care providers, our relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date services are rendered.
Broken Appointments
Charges may also be made for broken appointments and appointments cancelled without 24 hours advance notice.
